BP releases report on the “casual chain of events” that led to the Horizon disaster

This morning, more than four months after the Deepwater Horizon Disaster in the Gulf, BP released an internal report that outlined the 8 failures that lead to the massive oil spill. BP took on some of the blame but managed to heap a fair amount onto cement contractor Halliburton and rig owner Transocean.
